CITY OF MUSKEGO, WISCONSIN CERTIFICATE OF APPROVAL OF THE
ISSUANCE BY THE COLORADO HEALTH FACILITIES AUTHORITY OF ITS
HEALTH CARE FACILITIES REVENUE BONDS
(AMERICAN BAPTIST HOMES OF THE MIDWEST OBLIGATED GROUP)
SERIES 2013
WHEREAS, we have been advised that on October 17, 2013, notice of a public hearing
was published in the Muskego Now, a copy of which is attached to this certificate (the “Notice”).
The notice was designed to apprise residents of the City of Muskego, Wisconsin (the “City”) of
the proposed issuance of the Bonds captioned above (the “Bonds”), and was published no less
than 14 days before the scheduled date of the public hearing.
WHEREAS, on November 12, 2013, a public hearing was held at the Muskego Room,
Muskego City Hall, W182 S8200 Racine Ave, Muskego, Wisconsin 53150, with regard to the
issuance of the Bonds. The hearing was conducted in a manner that provided a reasonable
opportunity for persons to be heard.
NOW, THEREFORE, based upon all of the foregoing, I HEREBY CERTIFY THAT:
1. The undersigned is the Mayor of the City, and as such is the highest ranking
executive official of the City elected at-large by popular vote of the residents of the City.
2. I have been informed of the purpose for which the Bonds are proposed to be
issued and it is my understanding the Project is located within the boundaries of the City.
3. The undersigned hereby approves of the issuance of the Bonds by the Colorado
Health Facilities Authority for the purpose stated in the Notice. The City shall never make any
payments on or in connection with the Bonds, payments on which are the sole responsibility of
American Baptist Homes of the Midwest.
