PCR1996063CORRECTED AMENDED RESOLUTION #P.C. 63-96 APPROVAL OF AMENDED BUILDING, SITE AND OPERATION PLAN FOR JANESVILLE MEDICAL GROUP BY SUMMIT HEALTH CARE FACILITIES FOR THE PROPERTY LOCATED AT S69 W15636 JANESVILLE ROAD Summit Health Care Facilities for Waukesha Memorial Hospital for the Janesville Medical Group depicting and amended building, site and operational (BSO) plan, and WHEREAS, A site plan was submitted on April 4, 1996 by feet to the existing 5,000 square foot facility for a total square footage of 20,500 square feet, and WHEREAS, Said BSO illustrates the addition of 15,500 square WHEREAS, When fully operational, employment at this location will total approximately 30 persons including doctors, nurses, technicians and support staff with regular working hours being from 9:00 a.m. to 7:OO p.m. Monday through Friday with urgent care services offered during the day on weekends, and character and includes earth tone horizontal wood siding, masonry columns and asphalt shingle roof, and WHEREAS, Exterior building treatment residential in WHEREAS, Adequate parking, drive and landscaped areas have been provided. NOW THEREFORE BE IT RESOLVED, That the Plan Commission approves of the amended Building, Site and Operation Plan dated 03-25-96 subject to the following: 1. All exterior pole lighting be no greater than 15 feet in height and have sodium vapor, down cast, cut-off fixtures. 2. All deciduous trees be a minimum 2.5 inches in diameter, all coniferous trees be a minimum of 6 feet in height. 3. Said plan be kept on file in the Building Department along with any subsequent changes to the same. 4. Approval of grading and drainage plans by Public Works Committee. 5. Installation of a cross access drive on property owner to the west giving minimum of 500 foot spacing from existing drive to east; and approval from Waukesha County for additional access. 6. All appropriate approvals for holding tank and water supply * 7. Fire department approval of drive access and fire stopping/suppression systems if required. expand existing. 8. Installation of accel/decel lanes on Janesville Road or showing additional plantings along Janesville Road and 9. Submittal for Staff approval of revised landscaping plan plantings at sign base. 10. Submittal of landscape bond equal to twice the value of landscaping (including paving and yard areas) if occupancy is requested prior to installation. BE IT FURTHER RESOLVED, That all aspects of this building, site and operation plan as they relate to the building, parking maintained in perpetuity unless otherwise authorized by the Plan and drive areas, landscaped areas and signage shall be Commission. BE IT FURTHER RESOLVED, That failure to comply with the conditions of th'is approval shall result in the imposition of fines of $100 per day per violation, the initiation of legal action, or both. BE IT FURTHER RESOLVED, That a color rendering will be required to be brought forth to the Plan Commission for approval and a change in the current access point identified and any substantial changes brought about to the site circulation due to fire code requirements be brought forth to the Plan Commission for approval.
