APPROVAL OF COMMUNITY BASED RESIDENTIAL FACILITY FOR

APPLEWOOD MANOR AT S80 W16995 DLYNN COURT
(MEADOW GREEN SUBDIVISION)
Services for a description of any specific hazards which may
affect the health and safety of the residents of a Community
Based Residential Facility at 580 W16995 Dlynn Ct. has been
received, and
WHEREAS, A request from the Department of Health and Social
WHEREAS, The Department has requested that any natural or
man-made hazards be identified by the Plan Commission which may
affect the health and safety of the residents of the proposed
facility, and
WHEREAS, This proposed facility is is classified as a B-LS:
Infirmed Aged facility.
THEREFORE BE IT RESOLVED, That the Plan Commission of the
City of Muskego has reviewed the proposal for a Community Based
Dlynn Court in regard to the "EXAMPLES OF POTENTIAL HAZARDS" as
Residential Facility for the property located at 580 W16995
specified on Form DCS-290 and have not identified any specific
natural or man-made hazards which may affect the safety of the
residents.
Commission do not consider this finding to place any liability
on the City.
BE IT FURTHER RESOLVED, That the City and the Plan
