RESOLUTION #233-2006
. RESOLUTION TO ENTER INTO AN AGREEMENT WITH DIVERSIFIED BENEFIT
SERVICES FOR THIRD PARTY ADMINISTRATOR SERVICES
WHEREAS, The City desires to set up health reimbursement accounts for
employees/retirees of the City in an effort to control costs related to health insurance
premiums, and
WHEREAS, The City must do this by establishing a Section 105 plan under the IRS
code, and
WHEREAS, The City has determined it prudent to contract with a third party
administrator to administer the Section 105 plan, and
WHEREAS, The Finance Committee has recommended to the Common Council (based
on a recommendation from staff through an interview/proposal process) that the City
contract with Diversified Benefit Services for third party administrator services.
NOW, THEREFORE, BE IT RESOLVED That the Common Council of the City of
Muskego, upon the recommendation of the Finance Committee, authorizes the Mayor to
enter into an agreement with Diversified Benefit Services for 2007 based on the
following fee schedule:
. Plan Implementation - $3.00 per employee, per year
Monthly Administrative Fee per participant per month - $4.10
BE IT FURTHER RESOLVED That the Mayor and Clerk-Treasurer are authorized to
sign the necessary documents in the name of the City.
