WHEREAS, The City advertised for bids for the Southeast Water Main Extension and
the following base bids were received:

WHEREAS, The Public Utilities Committee and City Engineers have reviewed the bids
and recommend that the low bid submitted by Mainline Sewer in the amount of
$918,829 for the Southeast Water Main Extension Project be accepted.

NOW, THEREFORE, BE IT RESOLVED That the CommC?n Council of the City of
Muskego, upon the recommendation of the Public Utilities Committee, does hereby
accept the low bid of Mainline Sewer in the amount of $918,829 for the Southeast
Water Main Extension Project subject to approval of the DNR and PSC.
BE IT FURTHER RESOLVED That the Mayor and Clerk-Treasurer are hereby
authorized to sign the necessary documents in the name of the City, and are further
authorized to sign any changes at their discretion upon recommendation of the
Engineering/Building Inspection Director.

Bids for the above project were opened and read on April 22, 2004 at 11 :00 a.m. at the City
of Muske go City Hall and were as follows:

We have reviewed the documentation submitted by the apparent low bidder and found that:
1. The Bid Form has been appropriately completed.
2. The proposed major subcontractors and suppliers must be supplied for review.
We recommend that Mainline Sewer & Water, Inc. be awarded the Southeast Water Main
Extension Project / MW-36-04 contract, contingent on review of the major subcontractors
and suppliers, and WDNR and the Muskego Director of Engineering and Inspection approval,
in the amount of$918,829.00. This amount is based on unit prices and estimated quantities.
Actual quantities, and therefore the final contract price, may vary.
