RESOLUTION #I 12-2002
APPROVAL OF FLEXIBLE BENEFITS PLAN
WHEREAS, The City of Muskego wishes to adopt a cafeteria plan within the context of
Section 125 of the Internal Revenue Code for the benefit of the employer's eligible
employees.
NOW, THEREFORE, BE IT RESOLVED That the Common Council of the City of
Muskego, upon the recommendation of the Finance Committee, hereby adopts the
attached Flexible Benefits Plan (consisting of the flexible benefits plan document, the
Adoption Agreement, and component benefit plans and Policies) for the City of
Muskego.
BE IT FURTHER RESOLVED That the Mayor and Finance Director shall be designated
as Co-Administrators of the Plan and are hereby authorized to sign the necessary
documents in the name of the City for the implementation of said Plan pending review
of the Mayor in consultation with the City Attorney.
BE IT FURTHER RESOLVED That the Plan shall be effective June 1,2002 and that
after January 1, 2003, the City will combine the Plan with the existing Premium Only
Cafeteria Plan.
