AUTHORIZE EXECUTION OF DENTAL INSURANCE CONTRACTS FOR 2000
WHEREAS, The City of Muskego presently has contracts with First Commonwealth and
Compcare Health Services Insurance Corporation (Dentacare) to provide dental
insurance to subscribers who elect to participate totally at their own expense; and
WHEREAS, The City wishes to enter into contracts with First Commonwealth and
Compcare Health Services Insurance Corporation for dental insurance for 2000 at the
following monthly rates:
First Commonwealth
Intermediate Plan
UCR3 Plan
Sinale
(Network) $17 78 $47.41
(Out of Network) $20.12 $55.54
Compcare
Dentacare 160 $1 5.47 $41 77
WHEREAS, The Finance Committee has recommended that the City enter into
contracts with both carriers for 2000.
NOW, THEREFORE, BE IT RESOLVED That the Common Council of the City of
Muskego, upon the recommendation of the Finance Committee, does hereby approve
the City entering into contracts with First Commonwealth and Compcare Health
Services Insurance Corporation (Dentacare) for dental insurance for 2000 to be fully
funded by the subscriber
BE IT FURTHER RESOLVED That the Mayor and Clerk-Treasurer are authorized to
execute any necessary documents.
