AUTHORIZING EXECUTION OF AGREEMENT BETWEEN
SUN LIFE OF CANADA AND THE CITY OF MUSKEG0
WHEREAS, the City's present group life insurance carrier, North
American Life Insurance Co., has advised that as of October 15,
1986, it will no longer write the group life business for the
business, and
City as the Company has decided they are going out of the group
WHEREAS, the City's life insurance Agent has obtained the quotes
for Group Life and Accidental Death and Dismemberment coverage,
and
WHEREAS, the Finance Committee has reviewed these quotes and has
recommended the low quote of Sun Life of Canada at a monthly rate
per $1,00O/Mo. of .316.
NOW, THEREFORE, BE IT RESOLVED that the Common Council of the
City of Muskego, upon the recommendation of the Finance Committee
does hereby authorize the Mayor and City Clerk to sign the
necessary documents to execute the agreement between Sun Life of
Canada and the City of Muskego at a monthly rate per $1,00O/Mo.
of .316.
