AUTHORIZATION TO EXECUTE CONTRACT
(SAMARITAN HEALTH PLAN)
BE IT RESOLVED that the Common Council of the City of Muskego,
upon the recommendation of the Finance Committee, does hereby
authorize the Mayor and Clerk to execute the attached Group
Cooperative and the City of Muskego for 1986 with the following
Letter of Agreement between the Samaritan Health Plan
rates effective January 1, 1986:
Single Plan $ 84.62
Family Plan 216.38
