APPROVAL OF CONTRACT BETWEEN WAUKESHA COUNTY DEPARTMENT
OF AGING AND CITY OF MUSKEG0
WHEREAS,the Waukesha County Department of Aging has been using
the Denoon Senior Center as a location to provide food services
to the elderly, and
WHEREAS, it is necessary that an agreement between the City of
Muskego and the Waukesha County Department of Aging be executed,
and
THEREFORE, BE IT RESOLVED that the Common Council of the City of
Muskego, upon the recommendation of the Finance Committee, does
hereby authorize the Mayor and Clerk to execute an agreement
between the City and Waukesha County Department of Aging for a
nutrition program being conducted at the Denoon Senior Center for
the year 1984.

COOPERATIVE AGREEF.IE!.IT BETUEEN Denoon Senior Center AND
blAUKESHA COUNTY DEPARTMENT OF AGING
In consideration of the mutual covenants herein, klaukesha County Department
of Aging. State of !.!isconsin. hereinafter known as the "bogram," having offices
at 25942 1.1. :,lorthview goad, I*!aukesha, \I1 53185 and &noon Senior Center
located at 11185 SO912 Racine Avenue
Muskego I , !,!isconsin, 53150 , hereinafter known as'
the "Facility," agree as follorvs:
1. The Facility sball provide:
a. Use of the dining and serving area for service of mals on
!.Ionday, Wednesday, Friday from 19:30 a.m. to
1:30 p.m. for elderly participants.
b. Payment of utility charges.
c. Storage facilities for equipment and utensils.
d. Tables, chairs, and use of kitchen faci!:lies.
2. The Program shall provide:
a. !leals which shall be delivered and shall contain 1/3 minimum daily
requirenents for an adult.
b. Recruiting. training and schedulinq of volunteers who shall be res-
ponsible for hosting, serving, cleanun, record keepinq, etc. and
shall follow health and sanitation procedures. Also. set up tables
for chairs and place back in storaqe area as needed.
c. Planninq and coordination of various social and recreational activi-
ties following meal service as agreed. with the approval of the
Facil i ty.

The Faclllty and the Program shall agree as follows:
a. Heal service shall be open to all residents of the United States
60 years of age or older.
b. Various rules and regulations regarding walk-ins may be established
as necessary ty the Facility in cooperation with the Program.
c. The parties hereto shall each maintain adequate liability insurance
coverage regarding the operation of the, program. A certificate of
said coveraoe shall be provided on demand.

24-Hour notice will be given for any activities scheduled after
the desiyated hours.
This agreement constitutes the entire agreement between the Facility
and the Program.
This agreement shall be effective as of January 1 19 93
and endin! @ecei-ber 31 , 132. However. either party may
at anytime during the life of the aqreement terminate this agreement by
y!*:ing thirty days notice to the other party of the intention to do so.
