APPROVAL OF HOLDING TANK AGREEMENT

WHEREAS, the City of Muskego has received a request. from
!dr. Dennis Giuliana to install a holding tank at S98
W12904 Loomis Rd. in the City of Muskego, and
within the .established policy of the City of Muskego
WHEREAS, this is an existing home and therefore falls
for holding tanks, and
WHEREAS, a permit from the State.Board of Health, the
execution of a holding tank agreement and.-a $l;OOO.OO is
required along with approval by the Common Council,
THEREFORE, BE IT RESOLVED that the Common Council of the
City of Muskego does hereby approve of the installation
of a holding tank by Mr. Dennis Giuliana on the property
located at S98 W12904 Loomis Rd.
BE IT FURTHER RESOLVED that this approval is subject
to the execution of the necessary agreement, the receipt
0f.a $1,000.00 bond and the receipt of a permit from
the State-Board of Health. ~ I
