TRANSFERRING PRESENT LIFE INSURANCE PROGRAM TU
WISCONSIN PUBLIC EKE'LOYEE'S GROUP' LIFE
INSURANCE PROGRAM
RESOLVED, by the C-on Council of the City of Muskego
that pursuant to the provisions of section 40.20 of the
Wisconsin Statutes such city hereby determines to be inc3.uded
under the group life insurance program provided by section
40.20 of the Wisconsin Statutes, for its eligib1.e 'personnel., end
BE IT rmRTHER RESOLVED, that the proper officers hre here-
with authorized and girectod to take all actions. and make such
deductions and submit suqh payments as ue required by the
Group Insurance Board of the State' of. Wisconsin pursuant to
section 40.20 of the Wisconsin Statutes, and
BE IT FURTHER RESOI,VED, that the City Clerk submit a
certified copy of this resolutlvn to the Director,of.the Group
Insurance Bureau of thg State of Wisconsin.
BE IT FURTHER RESOLVED that the submfttal of this, resolution
to the Directors of the Group Insurance Bureau of the State of
Wisconsin is subject to a review of rates of the present carrier.
