APPROVAL OF HOLDING TANK AGREEMENT

WHEREAS, the city has received a request from Orlo J. Zlensky
to install a holding tank at S91 W13253 Boxhorn Drive, in
the City of Muskego, and
WHEREAS, a permit from the State Board of Health is dependent
upon the approval of the City of Muskego, the Waukesha County
Board of Health and that a deposit of a surety bond in the
amount of $1,000.00 is made with the city to guarantee compliance
with the rules and regulations of the State Board of Health,
and
WHEREAS, the Waukesha County Board of Health requires city
approval before they will consider the request,
THEREFORE, BE IT RESOLVED that the Common Council of the City
of Muskego does hereby approve of the installation of a
holding tank for Orlo J. Zlensky on the property located
at S91 W13253 Boxhorn Drive.
BE IT FURTHER RESOLVED that the Mayor and City Clerk are
authorized to execute the standard performance bond and
holding tank agreement in connection herewith.
