RESOLUTION REGARDING ELIGIBILITY & FURTHER
DETAILS REGARDING GROUP LIFE INSURANCE PLAN
WHEREAS, the City is in the process of completing an
application with Lincoln National Life Insurance Company for a
group life insurance plan to cover full time employees and full
time elected officials of the City;
BE IT RESOLVED that the City Clerk-Comptroller shall
be authorized to sign the master application for such plan and
that the Mayor and City Clerk be authorized to sign the contract
on behalf of the City.
BE IT FURTHER RESOLVED that those eligible for parti-
cipation in said plan shall be those employees and appointed
officials who perform work or services for the City on a regular
ceive a regular monthly salary or hourly salary for such work or
weekly basis for a minimum of hours per week, and who re-
serv ices.
Elected officials. Those eliaible shall be the offices ~~ -
of City Clerk-Comptroller and City Assessor.
