APPROVAL OF CONTRACT BETWEEN ASSOCIATED
HOSPITAL SERVICE, INC. AND THE
CITY OF MUSKEG0
a WHEREAS, the contract with the Associated Hospital Service, Inc.
must be renewed for the yea? 1967-68, and
WHEREAS, provisions providing hospital coverage to employees
has been adopted by the Common Council pursuant to Ordinance 1/55,
and
WHEREAS, the Finance Committee has recommended that the contract
with Associated Hospital Service, Inc., be approved,
THEREFORE BE IT RESOLVED that the Common Council of the City of
Muskego does hereby enter into the agreement with Associated
Hospital Service, Inc. whereby they will provide Blue Cross
Series 2000 - 120 Day Full Hospital Contract - Surgical Care
Preferred Surgical-Medical Contract with First Day Medical,
Multiple Consultations and Intensive Care Amendment.
BE IT FURTHER RESOLVED that the Mayor and the City Clerk are
hereby authorized to execute the necessary agreement in the
name of the City.
