COMMON COUNCIL - CITY OF MUSKEGO
RESOLUTION #204-2008
AUTHORIZE EXECUTION OF DENTAL INSURANCE CONTRACTS FOR 2009
WHEREAS, The City of Muskego presently has contracts with First Commonwealth and
Compcare Health Services Insurance Corporation (Dentacare) to provide dental
insurance to subscribers who elect to participate totally at their own expense; and
WHEREAS, The City wishes to renew contracts with First Commonwealth and
Compcare Health Services Insurance Corporation for dental insurance for 2009 at the
following monthly rates:
Single Family
First Commonwealth (Guardian)
(FCW HMO)
(Network) $55.30 $145.95
(FCW PPO)
(Out of Network) $39.37 $108.73
DentalBlue (Anthem BCBS)
Dentacare 4DS $35.13 $ 94.85
WHEREAS, The Finance Committee has recommended that the City renew contracts
with both carriers for 2008.
NOW, THEREFORE, BE IT RESOLVED That the Common Council of the City of
Muskego, upon the recommendation of the Finance Committee, does hereby approve
the City renewing contracts with Guardian and Anthem BCBS for 2009 dental insurance
to be fully funded by the subscribers.
BE IT FURTHER RESOLVED That the Mayor and Clerk-Treasurer are authorized to
execute any necessary documents.
