Upon the filing of the Complaint, a copy of which is attached hereto, the Common
Council of the City of Muskego authorized the issuance of this Sum m ons and you are
hereby com m anded to appear before the Com m on Council of the City of Muskego at a
tim e and date fixed by the Mayor pursuant to R esolution #______-2006 attached hereto
at the Muskego City Hall, W 182 S8200 Racine Avenue, Muskego, W isconsin and show
cause why your license should not be revoked and/or suspended based upon the
Complaint attached hereto, said time and date being fixed as the ______ day of
__________, 200__ at ______M.
If you do not appear as required by this S um m ons, the allegations of the com plaint shall
be taken as true, and if the Comm on Council of the City of Muskego finds the
allegations sufficient, the license shall be revoked.
If you appear as required by the Summons and deny the Complaint, both the
complainant and you, the license holder, may produce witnesses and cross-examine
witnesses under oath and be represented by counsel.
The hearing will be transcribed in writing and you have a right to a written transcript of
the hearing at your expense.
TO: Muskego Police Department: You are hereby directed to serve this Summons and
attached C om plaint upon Johnny’s P etroleum Products, Inc. The Sum m ons shall
command the licensee com plained of to appear before the municipal governing body on
a day and place nam ed in the Summons, not less than 3 days and not more than 10
days from the date of issuance. The Summons and a copy of the Complaint shall be
served on the licensee at least 3 days before the tim e at which the licensee is
commanded to appear. Service shall be in the m anner provided under Chapter 801 for
service in civil actions in circuit court.
