ACCEPTANCE OF PROPOSAL SUBMITTED BY AFLAC
FOR FLEXIBLE SPENDING CAFETERIA PLAN
WHEREAS, The City desires to initiate a Flexible Spending Cafeteria Plan for
employees; and
WHEREAS, The Finance Committee, in consultation with the City's Finance Director,
reviewed proposals submitted by Eflexgroup.com and AFLAC to perform the
reimbursement services of the plan for the City; and
WHEREAS, The Finance committee has recommended acceptance of the proposal
submitted by AFLAC at a monthly cost of $4.50 per participant with the company
waiving the initial set-up fees.
NOW, THEREFORE, BE IT RESOLVED That the Common Council of the City of
Muskego, upon the recommendation of the Finance Committee, does hereby accept
the proposal submitted by AFLAC to perform the reimbursement services of the City's
Flexible Spending Cafeteria Plan for 2002 at a monthly cost of $4.50 per participant.

Attached is the proposal that you requested in regards to the Flexible Spending Account
that we have been discussing. Included in the proposal are some basic facts about
AFLAC in general and the services that are offered at no charge to the City of Muskego.
I have also attached a Memo f?om AFLAC Administrative Services that states that they
will not only reduce the monthly participant fees from $5.00 per person to $4.50 per
person but, they will also waive the initial set-up fees if certain requirements are met by
the requested effective date of the plan. The memo states that the requested effective date
is June 1,2002.

FORM 5500 ASSISTANCE:
Upon request, based on information supplied by the. employer and provided that the. employer executes a
Form 5500 assistance release in favor of AFLAC, we will assist the employer in preparing the infodon
necessary for the Form 5500 filing obligabon by the IRS Section 6039D. The employer is solely
responsible for any and all 6hg and reporting obligations imposed upon sponsors of welfare benefit plans.
NON-DISCRIMINATION TESTING SERVICES:
Upon request, based on the information supplied by the employer and provided the employer executes a
nondiscrimination teshng release in favor of AFALC, we will assist the employer in determining testing
percentages for the cafeteria 25% concentration te$ the dependent care plan 5% test, and the dependent
care plan 55% average benefits test.
CAFETERIA PLAN DOCUMENTS:
Cafeteria plan documents will be provided by AFLAC to include the Adoption Agrement, Board
Resol~tion, Summary Pian Description and Reimbursement Services Agreement.
SALARY REDIRECTION AGREEMENTS:
Salary redirection agreements will be provided for employees to indicate participation or waiver of
participation.
EMPLOYER ADMINISTRATIVE MANUAL:
Employer administrative manual will be provided by AFLAC outluung Section 125 guidelines and plan
management guidelines.
NOTIFICATION OF CHANGES IN LAW:
Notification of changes in law affecting the administration or design of your program, including
amendments of plan documents if necessary will be provided by AFLAC.

Assistance is provided by technical information specialists trained to handle a variety of calls pertamq to
all aspects of Section 125 cafeteria plans. The technical information specialists are avarlable Monday
