RESOLUTION TO AMEND 1997 SUBGRANTEE AGREEMENT
BETWEEN WAUKESHA COUNTY AND MUSKEG0 HISTORICAL SOCIETY
WHEREAS, The attached represents a proposed amendment to CDBG Agreement RB-
1997-29 Between Waukesha County and the Muskego Historical Society, which was
executed in 1997; and
WHEREAS, The amendment modifies Exhibit A Scope of Work as previously identified
in the original agreement; and
WHEREAS, The Finance Committee has reviewed and recommended approval of the
Amendment.
NOW, THEREFORE, BE IT RESOLVED That the Common Council of the City of
Muskego, upon the recommendation of the Finance Committee, does hereby approve
the Amendment to the 1997 Subgrantee Agreement Between Waukesha County and
Muskego Historical Society, as attached.
BE IT FURTHER RESOLVED That the Mayor and Clerk-Treasurer are authorized to
execute the document in the name of the City.

Whereas, Waukesha County executed CDBG Agreement RB-1997-29 with the Muskego
Historical Society, and
Whereas, it is mutually agreed between the City of Muskego and Muskego Historical
Society to execute the amendment, and
Whereas, the parties are desirous of amending the Agreement to meet other specific
community needs, and
Whereas, the CDBG Board approved the requested modifications at its January 12,2000
meeting.
The following modifications to Exhibit A Scope of Work are hereby substituted for the
prior Scope of Work:

DEVELOPMENT ACT OF 1974 (PUBLIC LAW 93-383, AS AMENDED)
SUBGRANTEE: MUSKEGO HISTORICAL SOCIETY
CONTRACT mER: RB-1997-29
PROJECT TITLE: RESTROOM ACCESSIBILITY - OLD MUSKEGO SETTLEMENT CENTRE
A. SCOPE OF WORK
The purpose of the activities funded pursuant to this Agreement is to
develop a City historic center and purchase accessible hardware for an
accessible restroom facility.
The PROJECT will be undertaken at W182 58200 Racine Avenue, Muskego
53150.
The scope of the activities will be limited to the following:
1. Subgrantee, as part of the development of a City historic center,
will construct a restroom facility that is fully accessible for the
general public use.
2. Develop plans, speciEications and obtain City permits for
construction.
3. Subgrantee shall secure either through bidding process or by
securing volunteers the ability to construct the exterior shell of
a restroom facility.
4. Subgrantee shall identify all accessible related hardware including
but not limited to the following: bathroom sinks, toilets,
partitions, water fountains, door handles, ADA approved signage
etc. and submit list for CDBG Board approval of items
5. BIDDING: Obtain 3 purchase bids for items identified as accessible
related.
All work MUST BE BID in conformance with federal procurement
requirements as follows:
a. Work to be completed <$2.000 can be obtained through small bid
procedures. Three bids (minimum) should be obtained from
suppliers, contractors/business to obtain and award lowest bid.
b. Work estimated at $2,001 - $10,000 may be bid under competitive
negotiations. Specifications are developed and sent to a
minimum of three (3) contractors to obtain bids. This direct
solicitation of contractors should award the work to the lowest
qualified bidder
c. Work over $10,000 must be bid under formal bid procedures
including newspaper advertisement along with direct
solicitation with award to lowest qualified bidde,r
d. Procedure b and c must indicate that FH/EO and federal
prevailing wages are applicable in any bid document.
