CCR1985351RESOLUTION #351-85 APPROVAL OF CONTRACT BETWEEN WAUKESHA COUNTY DEPARTMENT OF AGING AND CITY OF MUSKEG0 WHEREAS, the Waukesha County Department of Aging has been using the Denoon Senior Center as a location to provide food services to the elderly, and WHEREAS, it is necessary that an agreement between the City of Muskego and the Waukesha County Department of Aging be executed, and WHEREAS, the Department of A ing will use the facilities in the lower level of the City Hall 7 Library building for the nutrition program. NOW, THEREFORE, BE IT RESOLVED that the Common Council of the City of Muskego, upon the recommendation of the Finance Committee, does hereby authorize the Mayor and Clerk to execute an agreement between the City and Waukesha County Department of for a nutrition program being conducted at the City Library building for the period of January 6 thru December 31, 1986. Waukesha County Department of Aging, State of Wisconsin. hereinafter known as the "Program." having offices at 25042 W. Northview Road, Waukesha, UI 53186 and -,, ,-f+v D - located at W182 58200 Racine Avenue -. 0 Muskego , Wisconsin, 353150 , hereinafter known as - the "Facility." agree as follows: 1. The Facility shall provide: a. Uie of the dining and serving area for service of meals on 9:30 Monday, Wednesday and Friday f.rom XiXWJ a.m. to &.m. for elderly participants. b. Payment of utility charges. - C. Storage facilities for equipment and utensils. d. Tables, chairs, and use of kitchen facilities. 2. The Program shall provide: a. Meals vhich shall be delivered and shall contain 1/3 minimum daily requirements for an adult. b. Recruiting. training and scheduling of volunteers who shall be res- ponsible for hosting. serving, cleanup, record keeping, etc. and shall follow health and sanitation procedures. Also, set up tables for chairs and place back in storage area as needed. C. Planning and coordination of various social and recreational activi- I ties following meal service as agreed, with the approval of the Faci 11 ty. 3. The Facility and the Program shall agree as follows: a. Meal service shall be open to all residents of the United States 60 years e of age or older. b. Various rules and regulations regarding walk-ins may be established as necessary by the Facility in cooperation with the Program. C. The parties hereto shall each maintain adequate liability insurance cover- age regarding the operation of the program. A certificate of said coverage shall be provided on demand. 4. Miscellanous terms: a. The nutritionprogram will be responsible for the disposal of trash in the appropriate receptacle as designated by the City of Muskego. b. The refrigerator donated to the nutrition program will be secured when pro- gram is not in session. A key will be made available to the city hall for other group'sor organization's use; however, due to the nature of the nutri- tion program, food may not be stored overnight. a c. The nutrition program's steam table will be made available to other groups utilizing the community room, pending prior approval by City Hall designee. 5. This agreement constitutesthe entire agreement between the Facility and the Program. 6. This ageement shall be effective as of January 1, 1986 and ending December 31. 1986. However, either party may at anytime during the life of the agreement terminate this agreement by giving thirty days notice to the other party of the intention to do so.
